Patient education, post-hospitalization follow up, chronic disease management support, and support for patients during care transitions, running and managing various reports identifying gaps in care and documenting medical information in the electronic health record.
The candidate for this position should be team focused, organized, and have excellent communication skills.
A clinical, health-related or community health education degree preferred.
